Your go-to for NDIS referrals

Connect someone with possibilities

An NDIS referral can be the beginning of more independence, new connections and meaningful progress.

Help us get to know the person you’re referring and what they hope support will make possible.

We welcome self-referrals and referrals from families, carers, guardians, support coordinators and other professionals. Complete the online form or download our participant referral form to get started.

Participant details

The information you provide will be used to respond to this referral and determine whether our supports may be suitable. Please see our Privacy Policy for information about how we collect, use and protect personal information.

Please note: Our NDIS registration is pending. All services are subject to individual assessment, funding, availability and a written service agreement.

We see the person and recognise potential

Their next chapter matters

Behind every referral is someone with their own experiences, abilities and ideas for the future. Tell us what matters to them, what’s working well and where they’d welcome a change.

When reviewing NDIS referrals, we consider the participant’s goals alongside their practical and clinical needs. Their communication preferences, culture, chosen support network and location all help us understand whether our team can provide suitable support.

Clinical knowledge and practical experience

Our director is a Registered Nurse, Clinical Nurse Consultant and Diabetes Educator, bringing experience in nursing, disability and community support to the assessment of care needs.

An understanding of regional communities

Based in Mount Isa, our team’s experience extends to remote North West Queensland. We consider location, service capacity and safe staffing when assessing each referral.

The participant’s perspective comes first

We seek to understand the participant’s own priorities, strengths and preferences, giving their voice a central place in discussions about suitable support.

Collaboration from the beginning

We work with participants and their chosen families, carers, guardians and support coordinators, bringing different perspectives together to plan support and clarify the next steps.
